Freedom of Information Act (FOIA) Form:

To request information under the Freedom of Information Act (FOIA), please complete the online fillable FOIA form Freedom-of-Information-Act-Form. You can also mail, fax, or drop the completed form off in person.

The cost to obtain copies is as follows: No fees shall be charged for the first 50 pages of black and white, letter, or legal-sized copies requested. After the first 50 pages, the fee for black and white, letter, or legal-sized copies shall be 15¢ per page. Actual cost will be charged for documents not of standard size and for the recording medium (e.g., compact disk, tape, DVD). Fees may be waived or reduced if the person requesting the records states the specific purpose for the request and indicates that a waiver or reduction of fees is in the public interest.

Response to Request: The Police Department will disclose the public records requested on the online FOIA Request Form within 21 Business Days after the receipt of this Request Form for all requests made for commercial purposes and within 5 Business Days for all other requests unless the applicable response period is extended as provided by law or the request is denied. All extensions and denials will be in writing and will state the reasons. The Police Department will not be responsible for any Request not received by the Police Department due to error or delays beyond the reasonable control of the Police Department. The Requestor may seek a review of a denial by the Public Access Counselor of the Office of the Illinois Attorney General. Judicial review is available under Section 11 of the Illinois Freedom of Information Act, 5 ILCS 140/1 et seq.
